These prompts are posted to give a preview of the Final Survey form for Faculty Grants (Team Grants). We strongly encourage mentors and team members to look ahead to the deadlines posted here, review the survey prompts and prepare responses in advance. Faculty Grant recipients will start the process by submitting their portion of the Team Final Survey (using the link below), then their team members will receive email notifications with unique links to submit their portion of the survey. Team mentors won't receive email confirmation when their team members submit the survey, but we invite mentors to contact us with questions.
